1. Definition 

 The definition of a questionnaire according to Dewa Ketut Sukardi, a questionnaire is a form of technique or tool in data collection that is carried out in research methods without the need or obligation to present the data source directly.

2. Function

The function of the questionnaire is :

 1) To guarantee the validity of the information obtained by  other methods. 

 2) Making program evaluations.

 3) Taking sampling or opinions from the respondents.

 4) Collect information from the general public

3. Types

 1)Computer questionnaire. Respondents are asked to answer the questionnaire which is sent by mail.

 2)Telephone questionnaire. Researcher may choose to call potential respondents with the aim of getting them to answer the questionnaire.

 3)In-house survey. This type of questionnaire involves the researcher visiting respondents in their houses or workplaces.

 4)Mail Questionnaire. This sort of questionnaires involve the researcher to send the questionnaire list to respondents through post, often attaching pre-paid envelope.

 5)Open question questionnaires. Open questions differ from other types of questions used in questionnaires in a way that open questions may produce unexpected results

 6)Multiple choice questions. Respondents are offered a set of answers they have to choose from. 

 7)Scaling Questions. Also referred to as ranking questions, they present an option for respondents to rank the available answers to questions on the scale of given range of values (for example from 1 to 10).

4. Expression

 1)Allegation regarding a person or persons

 2)Allegation regarding a medium of communication

 3)Information regarding the alleged perpetrators

 4)Information related to State actions

 5)Information on the source of the communications
