1.      Definition

 Survey or complete self-administered survey is a method of collecting primary data by providing questions to individual respondents. So, it can be concluded that a survey is a method of collecting information from a group that represents a population: A large number of respondents.

2.      Function

  The purpose of the survey is to expose the data of the object of study, and interpret and analyze it systematically. The correctness of the information depends on the method used in the survey.

The uses of the survey include:

1)     To derive facts from existing symptoms;

2)  Seeking factual information from a group, region etc.

3)   Evaluating and assessing what others have done in dealing with similar matters;

4)    Conducted on a number of individuals/units either by census or sample; and

5)   The results are for plan making and decision-making.

There are several columns, the first column there is an example column, the second to 6th is the scale of how often the site is opened, and the last column is an option if it never opens the site.
